Have you had a positive Covid test in the last 4 weeks? *
Have you had symptoms of Covid (temperature, new cough or a change in sense of taste/smell) in the last 4 weeks? *
Have you had any other vaccine in the past week, or will be having one in the next few days? *
Do you have a serious allergy that requires you to carry an Epipen (adrenaline injector)? *
Would you like to have the Covid vaccination? *
Would you be able to travel to the vaccination centre in Wokingham? *
Please read the following information carefully, and indicate that you have understood it by ticking the box below:
- On the day we are asking people to arrive via the main entrance to the Bradbury Centre, off Peach Place. Parking is available in Rose Street car park (old M&S), and marshals will be present to show you where to park
- Only people who are being vaccinated will be able to come into the clinic unless you need assistance inside the clinic, in which case 1 person may come inside with you. If other people come with you, or bring you to get vaccinated, they will need to wait outside.
- Please wear a face covering when you attend for your appointment and observe social distancing when you are waiting for your appointment.
- Please arrive as close to your appointment time as possible as there is no indoor queuing facility and the weather will be cold. We want to keep you out of the cold as much as possible.
- There will be a period after vaccination that you will need to sit and be observed in case of any reaction. Please bring a pen with you as we will be collecting some information from you to help keep your medical records up to date.
- There will be a second dose of the vaccine after 12 weeks – you will be contacted again when we have more information.
